Vital Statistics

Birth Certificates           

Birth Certificates

First Copy   $12.50 each

Second or Additional Copies $8.00 each

Birth records are available from 1930 until present for State of Florida births.  They can only be released to authorized persons (i.e. a parent, legal guardian, etc.).  The recipient must be 18 years of age or older and present a valid picture id, such as a Driver's License, State Issued Identification Card, a passport, and/or a Military Identification Card.

Death Certificates  

$10.00 each

Death records are available from 2000 until present if the death event occurred in Taylor County.  If the death occurred prior to 2000, please contact the State of Florida's Office of Vital Statistics at (904)359-6900. 

Certification of death certificates that do not contain the medical certification of cause of death may be issued to any applicant 18 years of age or older upon receipt of a written request and the required fee.

A certification of a death certificate including the cause of death information may be issued only to the decedent's:

1.  Spouse or parent

2.  Child, grandchild, or sibling, if of legal age; or

3.  To any person providing a will, insurance policy, or other document demonstrating his or her interest in the estate of the decedent, or

4.  Any person who provides documentation that they are acting on behalf of any of these persons; or

5.  Upon order of a court of competent jurisdiction; (s.382.025(2), F.S.)

Additionally, the recipient must be 18 years of age or older and present a valid id, such as a Driver's License, State Issued Identification Card, a passport, and/or a Military Identification Card.

Copies of Medical Records

Due to confidentiality restrictions, copies of medical records are released only after a consent to release records has been signed and submitted.    There is a $1.00 per page fee to all parties except the patient themselves.  A faxed request will be accepted from other medical providers. The fax number is (850)584-7335.  Only general medical information will be supplied.  Any super-confidential (HIV, STD or mental health records) has to be specifically requested.  Please allow 10 days for return of medical records.
