Primary Care:
The Primary Care
program offers the following services: Medial history, physical exam,
screening, some prescription assistance, health education, well and sick care
and medical follow - ups when a physician or other medical provider is
available. This service is available at no cost or low cost, based upon
eligibility. There is no cost to Medicaid participating clients.

Chronic Disease
Program:
There is some
assistance for diabetic and epilepsy medications. Please call (386)
