         1930 through present-Statewide
      1912 to present for Walton County Births
 

   Fee for Birth Certifications:
         $12.00

    Issued only to the person
          named on the record when 18
          years of age or older, or to the
          registrant's parent, legal
         guardian

     Legal guardian must show
          guardianship papers
     Must have a picture ID
     Payment in cash,  check, Visa or 
           MasterCard

        $10.00
   Must be a family member of legal
        age or an attorney        
   If cause of death requested an ID
        must be presented
