Adult Health

Services

The Bradford County Health Department offers the following Primary Care services.

  • Medical History

  • Physical Exam

  • Screening

  • Limited Prescription Assistance

  • Health Education

  • Well & sick care visits

  • Medical Follow-ups when a physician or other medical provider is available.

Eligibility

This service is available at no cost or low cost, based upon eligibility. There is no cost to Medicaid participating clients.  Please call (904) 964-7732 to speak to an interview clerk for further information. Proof of household income is required such as pay stubs, income tax returns, social security income, etc.

 

Cost

Services are based on ability to pay (sliding fee scale).

 

Chronic Disease Program:

There is some assistance for diabetic and epilepsy medications.  Please call (904) 964-7732 for further information.
