I.

Repair OSTDS Application for property  ($185.00 fee for application and permit)

Need completed OSTDS application with the following:
1. Legal description with metes and bounds or survey of property
2. Floor plans of home:  (may be drawn by applicant)
3. Site plan drawn to scale if less than 5 acre.  If over 5 acre you do not have to draw to scale, but all dimensions must be written in showing the following items:
  • OSTDS location and direction of drain field lines
  • well location and direction of waterline from well to home 
  • location of home
  • driveway
  • easements
  • slopes
  • drainage ditch, lakes, ponds, canals
  • location of neighbors well and OSTDS
4. Pump out certification of tank required from registered Septic Tank Contractor (form 4015 page 4) if more than 3 years old.
5. Site plan attachment sheet (obtained from health department)
6. Land use letter from zoning department
7. Information on existing system must be located at the Health Department or existing system will need to be pumped by a licensed contractor and the following information provided:
  • Size of Tank (Contractor needs to denote dimensions of tank)
  • Size of Drain Field (Lines need to be marked for Inspector to verify).
  • Condition of the Tank
  • Verify if system has outlet filter device
NOTE:  If site evaluation is performed by a certified soil scientist, they need to mark on the site plan location of benchmark and initial.  They also will need to fill out the Site Evaluation and System Specifications Sheet.
