Birth Certificates

Certified Copies of Birth Certificates for births occurring in the State of Florida from 1917 to present are available to eligible applicants. You must be 18 or older to obtain your own birth certificate, or be the parent or legal guardian. A valid photo ID is required. The fee is $13.00 per copy. You may download an application from this web site, and apply in person or by mail. 

Death Certificates

Florida Electronic Death Records began in late 2011. If the record was filed electronically, it is available at any Vital Statistics office in Florida. If a paper record was filed and the death occurred in Clay County within the past two years, you may obtain a copy from our office. All other Florida deaths must be obtained from the State Office in Jacksonville. Anyone may obtain a certified copy of a death certificate, but only the immediate family may obtain a certified copy with the cause of death. Valid photo I.D. is required when requesting the cause of death. The fee is $10.00 per copy. You may download an application from this web site, and apply in person or by mail.
