About the WIC Program |
WIC is a federally funded nutrition program for Woman, Infants and Children. WIC provides the following at no cost:
Vision: Healthy women and children through effective nutrition education and services, nutritious foods, and referrals to appropriate health and human services, delivered with integrity and respect for those we serve. Mission: Deliver the highest quality services in the nation with integrity and respect to 100% of funded potential WIC eligible population as measured by health outcomes. |

WIC Eligibility
Eligibility is by appointment only, for women who are pregnant, breastfeeding or have just had a baby and their children up to the age of 5 years old. There is a family income limit, but working families may qualify.

•To apply for WIC please contact our office at 904-272-3177 or visit our office to receive an appointment

•The appointment can generally take between one and two hours to complete.

•One-on-one nutrition counseling and assessment from a licensed nutritionist or qualified professional.

•Support and information for breastfeeding your baby.

•Information about nutrition and health to help you and your family eat well and be healthy.
- Checks to buy healthy foods including milk, cheese, eggs, juice, cereal, dry beans, and peanut butter.

•Referrals for other health care and community services.
What do I need to bring to my certification or recertification appointment?

•Identification for everyone that has an appointment (ex. photo ID, baby's crib card or shot record, social security card, etc, ) .

•Your child or baby that is being certified in WIC.

•Proof of income (ex. paystubs, etc), If you are NOT on Medicaid, temporary cash assistance or food stamps.

•Children’s shot record.

•Proof of address (ex. Drivers license, utility bill, etc.).

Your child's shot record.
- Your WIC card (if you are currently participating in WIC) .

What if my child’s doctor changes the formula?
WIC will need a WIC FORMULA REQUEST FORM to be completed by your child’s doctor, if the formula is something other than Nestle Goodstart or Goodstart Soy. You may get this form from your doctor or us. Bring it to us or fax it to us.
- This ENTIRE form needs to be completed, including a valid medical diagnosis and also the number of months the formula is requested for (the maximum is 6 months). - Even with a WIC Formula Request form from your doctor, there are some types of formula WIC may not be able to provide.
- If the request form is approved, checks and/or unopened cans of formula that you have, will need to be returned to WIC in exchange for the requested formula.
- Please remember that if your infant is in need of a special formula, he/she may need to be more closely monitored by WIC, and may need more frequent appointments.
