Social Services Department
All clients that would like to be seen at the Clay County Health Department are directed to come in for a social services assessment. This allows our staff to determine eligibility for clinic services, qualify clients for our sliding fee scale and make referrals to community services and programs that they may not be aware of. Please review the social services checklist to ensure you bring the appropriate documentation or it may delay your approval for services at a reduced cost.
Counselors are available to assist with the following applications:
Social service assessments are done on a walk-in basis, Monday through Friday, between 8:00 and 10:30am, or between 1:00 and 2:30pm.
Additional programs managed by this department include:
Tomorrows Rainbow: Grant program for uninsured women 50 to 64 that qualify financially to pay for PAP and mammograms
Susan Komen: Grant program for uninsured women under age 50 to pay for screening and diagnostic mammograms
WeCare: Provides specialty referrals for clients at or below 150% of the federal poverty level that have no other resources
Diabetic Supplies: Provided at a reduced rate to clinic patients
Additional programs provided by other state agencies
