Eligibility Requirements
Current Ryan White Notice of Eligibility:
Gross income less than or equal to 400% of Federal Poverty Level
Proof of HIV
Proof of living in Florida
Proof of income
Proof of any third party insurance (Medicaid, Medicare, Private Insurance,
Veterans Benefits etc.)
Must have at least one prescription for a medication on the ADAP formulary
Must have an HIV viral load and CD4 test result less than 6 months old

AIDS Insurance Continuance Program (AICP)
Services
The AIDS Insurance Continuation Program (AICP) is a statewide program for
persons who are diagnosed with AIDS or are HIV positive with symptoms and
who, because of their illness, are unable to maintain their private health
insurance coverage
AICP will pay up to $750 per month for premiums, $1,200 per year for
co-payment assistance, and $2,500 a year for services under deductible
The AICP does not pay for disability or life insurance
The AICP will also pay any fees associated with conversion of a COBRA policy to an individual policy or policy upgrades

Clients eligible for Medicaid/Medicare coverage are not qualified for enrollmentin AICP.

Oral Health Educator (Tooth Fairy)
The Tooth Fairy Program is part of the Collier County Health Department. The purpose of the Tooth Fairy/Oral Health Educator is to promote the health and wellbeing of the younger residents of Collier County as well as their parents. By introducing oral health to children in a light-hearted and educational manner, the CCHD strives to promote health care standards with the TOOTH FAIRY as Ambassador of Oral Health. Eligibility Requirements
Clients must bring passport and/or parole card and/or immigration documents to register for an appointment. Copies of immunization and medical records, medications, and x-rays should be brought to your first appointment as well.
Fees or Payment
Refugee physicals and immunizations are free for those who qualify under the Refugee Resettlement program
Immigration physicals $175.00 (adult) $115 (child) - includes exam, PPD,
syphilis testing and I-693 forms. Individual vaccine fees apply.

Project AIDS Care Waiver (PAC)
Services
PAC waiver provides home and community-based services to Medicaid eligible persons with a documented diagnosis of AIDS (Acquired Immune Deficiency Syndrome) that choose to live at home and in the community.

Recipients of PAC waiver services must demonstrate health conditions, such as the presence of AIDS related opportunistic infections, and limitations in functioning that cause them to be at risk of hospitalization or placement in a nursing facility, were it not for the provision of PAC waiver services.

PAC waiver service enables recipients to obtain access to needed medical and social support services regardless of the funding source. Case management identifies, organizes, coordinates and monitors services needed by the recipient.
