Adults 19 and older. Please bring documentation of past vaccines if available.

Fees or Payment

Individual vaccines prices will apply
Medicare, Medicaid, private insurance, private pay. We will bill your insurance
Co-payments and payments for vaccines not covered by insurance are due at time of service
Vaccine patient assistance programs are available for some vaccines for those who meet financial eligibility. Applications can be processed during your visit.

$40 nursing assessment fee unless client is scheduled for immigration
physical with the health department
