Basic Dental Services for Children
The clinic is available to children with Medicaid who live in Pasco County, ages 1 through 20.
Beginning in July 2008, we will also be accepting uninsured children ages 1 through 17. Payment will be on a sliding fee scale basis.

Questions:

Q:   Who can be seen in the dental clinic?

A:   Unless you are already a current dental patient, you must be one of the following:

- A Pasco County resident, age 1 through 20, with full Medicaid coverage
- A Healthcare Connections client with appropriate Dental Referral from Physician or Case Manager
- A child, age 1 through 17, who lives in Pasco County and has NO Dental Insurance or Discount Plan
     (Eligibility is based on household size and total household income)

Q:   How do I know if my child is eligible?

A:   You may stop by our clinic Monday ? Thursday to pick up the Eligibility Form.

You must bring the following items:

- Photo ID with current address (must be a Pasco County resident)
- Social Security Cards for ALL family members
- Proof of  ALL Household Income (one month?s worth)
ie: wages, salary, disability, unemployment, Social Security, pension, or any other means of support

PLEASE NOTE: If your child has any type of dental coverage (other than Medicaid), either through Kid Care, parents? employment or self-purchased, they are not eligible for dental care at this clinic. This includes Dental Discount Plans.

Q:   Does it cost anything?

A:   If you or your child are under age 21 and have Florida Medicaid, there is no fee for dental services. Your Medicaid pays for dental, regardless of which provider/plan you have chosen

- If you are a Heathcare Connections client you do not have to pay for dental services
- If your uninsured child(ren) is/are eligible for this dental clinic you will be asked to pay $25.00 for each visit

Q:   What dental services do you offer?

A:   We offer Exams, X-Rays, Cleanings, & Fillings (both silver and tooth-colored)


- If an extraction is needed, our dentist will determine if it can be done in this clinic

Q:   When are you open?

A:   Monday thru Thursday from 7:30 am to 5:30 pm

We are usually closed from 12:00 noon to 1:00 pm for Lunch
- We are CLOSED on Friday, Saturday, & Sunday

A:   Dental services are only provided at the following locations in Pasco County

Clients with referrals from Healthcare Connections Clinic programs will be accepted.
