Vital Statistics
Vital Statistics consists of official records of birth, death, marriage and dissolution of marriage. These records are essential for just administration of our law and for the protection of individual rights. In addition, the statistical data from these records are of great value to public health and other agencies. You may need certified copies of these records for:

Records Available at the Santa Rosa CHD
You may purchase two types of certified documents from our local office:

Certified copies of Birth Records (for anyone born in the state of Florida)
-
Death Records (if the death occurred in Santa Rosa County)
These may be obtained through the Vital Statistics office at Santa Rosa County Health Department, in person or by mail or online through a third party. See Below for online ordering. Our locations, phone number and hours of operation are:

Please note: at our Midway location, only birth certificates will be available on a walk-in basis. Death certificates for individuals who died in Santa Rosa County can be ordered at the Midway location and will be mailed to the applicant or can be picked up the next business day.
(We are closed on weekends and state holidays)
Proper Identification
Proper identification (ID) is required for all requests for birth records and death records with cause of death, whether the request is walk-in or mail-in. This is to determine that the applicant is, in fact, the person he/she purports to be. Some acceptable forms of ID include:

Driver's license

State ID card (issued by DMV)

Passport
-
Military ID
Remember, if a mail request does not include the proper ID, the request will be returned to the applicant.
Ordering Records Online
At this time the Health Department in Santa Rosa County does not accept online orders for birth or death records. However, online requests can be processed through the Bureau of Vital Statistics. For more information please visit http:\\floridavitalstatisticsonline.com.
