Population Based Services


 
 
  • Epidemiology
    Our Epidemiology nurse provides active tuberculosis case management, and active and passive disease surveillance activities for Walton county.  Chief responsibility to investigate reported cases of communicable disease and maintain an active reportable disease surveillance program.
     

  • Healthy Start
    The goal of Healthy Start is to reduce infant mortality, reduce the number of low birth weight babies, and to improve health and developmental outcomes.  Our staff provide care coordination, case management, education and referral, follow-up to women and teenagers during pregnancy, and to infants after delivery.
     

  • Mom Care
    Mom Care assists women who receive Medicaid because of pregnancy. We help to find a physician who accepts Medicaid, assist with arranging transportation to doctor's appointments, making a WIC appointment, and receiving a Healthy Start Screening.

  • School Health
    School Health services are provided in Walton County as a collaborative effort in the Walton County School District and the Walton County Health Department.  The school health planned strategies are to implement activities mandated by law and to ultimately improve the health of Walton County students and improve student success in school.
     

  • Healthy Lifestyles
    An educational pilot program on making healthy food choices, increasing physical activity and achieving a healthy weight.
