Communication Development Monitoring Reporting Form

Communication development monitoring is conducted by early intervention service providers for children with hearing loss eligible for Early Steps.  Not all children learn language at the same rate and it is important to know how quickly they are learning. The primary purpose of communication development monitoring is to inform the parents of their child's language and auditory skill development progress over time and provide them with the opportunity to consider if any changes are needed to the child's current level of communication access. The Communication Development Monitoring Reporting Form allows early intervention service providers to enter communication development information on children eligible for Early Steps.
