Listed below are the forms used for counseling and testing. Please note that county health departments should continue to supply forms to community-based testing programs.

DH 1628 Laboratory Request Forms

Each county receives an allocation based on the number of tests that were done the previous calendar year. If a county requests additional forms, data will be run on the number of tests performed to date and compared to the number requested. If the county is using more forms than their original allocation, more will be provided.

DH 1628 forms are housed at the Distribution Center and cannot be ordered directly through SIMS. Each county should have at least one contact person who orders forms for the entire county, including non-CHD sites. These county representatives are the only ones who may order this form.
