Reporting

Florida is conducting public health surveillance in order to obtain reliable and timely information on the incidence and prevalence of ALS, to better describe the demographic characteristics of ALS, and to assist ATSDR in evaluating the completeness of the National ALS Registry.

The FDOH is initiating a statewide ALS surveillance project. Every neurologist who has diagnosed or provided care to a patient with ALS from January 1, 2009 to December 31, 2011 is being asked to participate.

Florida has a very broad public records law. Most written communications to or from state officials regarding state business are public records available to the public and media upon request. Your e-mail communications may therefore be subject to public disclosure.
