MCH Block Grant Webpage Each year, states apply for funding from the federal government. This application includes a report on activities during the previous year as well as plans for the next. The report tracks state and national performance measures. Other forms include budget details, individuals served, and births by race. |

Survey of MCH Needs - Final Report -
[pdf; 587kb] As part of the statewide five-year needs assessment conducted for the FY2006 MCH Block Grant application, the Department of Health contracted with Florida State University to conduct a survey of MCH clients/consumers, providers, and key informants. This report details the results of mail and Web-based surveys conducted in 2005. |
