If you have any expired specimen cards, please DISCARD them. Specimens collected on expired cards will be REJECTED.
 
ACCEPTABLE cards have the following lot numbers and expiration dates:
 
Lot W092 Exp Date: 2013-05
Lot W101 Exp Date: 2013-09
Lot W102 Exp Date: 2014-02
Lot W111 Exp Date: 2014-07
Cards with Lot number higher than W111 and expiration date later than 2014-07
 
Fax a request to Newborn Screening Laboratory if more cards are needed.
