Practitioner Profiling
Disclaimer

CONSUMERS ARE URGED TO READ THIS NOTICE PRIOR TO SEARCHING THE DATABASE.

The practitioner profile summarizes data submitted to the Division of Medical Quality Assurance by Allopathic, Osteopathic, Chiropractic, and Podiatric Physicians and Advanced Registered Nurse Practitioners as required by law.  Some of the information has not been verified by the department.  The practitioner is responsible for updating the profile and adding new information to the profile. Thus, neither the Department of Health nor the State of Florida accepts any legal liability or responsibility for the accuracy, timeliness, completeness, or usefulness of this information.

The department verifies criminal history by conducting statewide criminal background checks every two years, immediately following a renewal cycle for practitioners.  The department also verifies disciplinary actions and liability claims against the practitioner by conducting a search of the National Practitioner Data Bank.  The practitioner is required to list any disciplinary action or liability claim that has occurred in the last ten years.  Therefore, the practitioner may have disciplinary action and liability claims that occurred more than 10 years ago that are not published on the practitioner profile. Additionally, federal criminal history information is not available to the public and thus, is not listed here.

Please refer to the Guide to the Florida Practitioner Profile (pdf - 953kb) and check all identifying factors to avoid confusion with similar names when reviewing a practitioner profile.  Please also discuss information with the practitioner prior to making a decision based upon this data.

Inappropriate use of any information to make incomplete or misleading comparisons of physicians may violate the law.

You may obtain other public information by contacting the appropriate state or federal agency or by linking to other websites.

If you wish to obtain information on Medical Malpractice payments, please contact the Department of Financial Services.  Civil medical malpractice payment information may be obtained from the county in which the practitioner is working.
