specialty
- about the practitioner's current practice and mailing addresses
- about the practitioner's staff privileges and faculty appointments
- about the practitioner's reported financial responsibility
- about legal actions taken against the practitioner
- about board final disciplinary action taken against the
practitioner
- on any liability claims filed against Podiatric Physicians which
exceed $5,000
- on any liability claims filed against allopathic and osteopathic
physicians which exceed $100,000
