STATUS COMPARISON
Retired
- You must pay a one-time fee of $55.00
- The Board will no longer contact you regarding your license
- Retired status does not allow you to continue writing prescriptions
Reactivating Retired License
- You must submit a written request to convert your retired license to an active status
- Pay the current licensure fees, plus all past renewal fees that would have been imposed if your license was in an active status
- Submit proof of completing 20 hours of CME for each year the license was in the retired status
- Submit a Financial Responsibility form (pdf - 35kb)
- Submit proof that you are NICA compliant
- Take the SPEX exam if you have not practiced medicine in another jurisdiction for 2-years out of the immediate 4-years
Inactive
- You must renew your license every two years
- Complete required CMEs
- Pay the $250.00 annual assessment fee to NICA
Reactivating Inactive License
- You must submit a written request to convert your inactive license to an active status
- Pay the current licensure fees, plus all past renewal fees that would have been imposed if your license was in an active status
- Submit a license verification form from each state that you were issued a medical license
- Submit an affidavit listing your medical activities in any jurisdiction while your Florida medical license was in an inactive status
- Submit an affidavit attesting to no disciplinary action in another jurisdiction while your Florida medical license was in an inactive status
- Submit proof of completing 20 hours of CME for each year the license was inactive
- Submit proof that you are NICA compliant
- Take the SPEX exam if you have not practiced medicine in another jurisdiction for 2-years out of the immediate 4-years
