RETIRED AND INACTIVE LICENSE
STATUS COMPARISON

Retired

  • You must pay a one-time fee of $55.00
  • The Board will no longer contact you regarding your license
  • Retired status does not allow you to continue writing prescriptions

Reactivating Retired License

  • You must submit a written request to convert your retired license to an active status
  • Pay the current licensure fees, plus all past renewal fees that would have been imposed if your license was in an active status
  • Submit proof of completing 20 hours of CME for each year the license was in the retired status
  • Submit a Financial Responsibility form (pdf - 35kb)
  • Submit proof that you are NICA compliant
  • Take the SPEX exam if you have not practiced medicine in another jurisdiction for 2-years out of the immediate 4-years

Inactive

  • You must renew your license every two years
  • Complete required CMEs
  • Pay the $250.00 annual assessment fee to NICA

Reactivating Inactive License

  • You must submit a written request to convert your inactive license to an active status
  • Pay the current licensure fees, plus all past renewal fees that would have been imposed if your license was in an active status
  • Submit a license verification form from each state that you were issued a medical license
  • Submit an affidavit listing your medical activities in any jurisdiction while your Florida medical license was in an inactive status
  • Submit an affidavit attesting to no disciplinary action in another jurisdiction while your Florida medical license was in an inactive status
  • Submit proof of completing 20 hours of CME for each year the license was inactive
  • Submit proof that you are NICA compliant
  • Take the SPEX exam if you have not practiced medicine in another jurisdiction for 2-years out of the immediate 4-years
