Effective January 1, 2013: Applicants for initial licensure who are required to submit fingerprints for a criminal history background check must use a Livescan service provider. Applicants who are fingerprinted after January 1, 2013 will have their prints retained by the Florida Department of Law Enforcement. If you are applying for initial licensure after December 15, 2012, You must print this form and take it with you to a Livescan service provider. Application and License Requirements
Apply Online for Licensure: Eliminate mailing time and expedite your application! Apply online, using your user id and password, and receive a response within 10-13 business days!
Exam: All applicants are required to pass the National Board of Chiropractic Examiners Part IV and Physiotherapy Examinations prior to submitting an application. We will accept the passing scores deemed appropriate by the NBCE.
All applicants are also required to take the FL laws/rules computer based test (CBT) once their application is deemed complete, which will be offered year-round.
