For Medical (MD) and Osteopathic Physicians (DO)
You may request Licensure Verification through VeriDoc

For Nurses (PN or RN)
You may request Licensure Verification through Nursys.

Note: If you are no longer licensed in Florida, your license information may not be available through Nursys.

For Pharmacists and Pharmacist Interns
If you are a licensed pharmacist or pharmacist intern and need your intern hours transferred to another state, please submit a written request to the Department of Health, Florida Board of Pharmacy, 4052 Bald Cypress Way, Bin C-04, Tallahassee, FL 32399

For EMTs, Paramedics, and Radiologic Technologists
At this time, you may request Licensure Verification in writing only. There is no fee associated with Licensure Verification for your profession.

For All Other Health Care Professions
You may request licensure verification online or in writing. There is a $25.00 processing fee.

If you are applying for a Florida license, you do not need to submit a verification request for your Florida license.
