This repository is refreshed nightly Monday through Friday with data from the primary database. For real-time information about a practitioner, please visit the License Look Up website.
Scanned final orders and emergency actions that were filed prior to June 2006 are available by clicking on the final order number in the agency action reports listed on this page.
Search for disciplinary and administrative complaint information for a licensee using our license verification application. If the licensee you are looking up has discipline or an administrative complaint on file, it will be indicated in the “General Information” tab, with a “Yes” under “Discipline on File” and/or a “Yes” under “Public Complaint”.

If you are unable to obtain the information or documentation you need from the applications above, you may submit a licensure public records request by contacting the Department at (850) 245-4121, completing the online form or submitting a request in writing (fees may apply). Public records requests are not required to be in writing. You do not have to give your name or explain why you are making a request.
