Q. How do I comply with my Final Order?
A. Please review our website and your Final Order. If you need further assistance, please email your Compliance Officer or contact the Compliance Management Unit at 850-245-4268.
Q. How do I obtain a copy of my Final Order?
A. You can search, view and print your final order using our Final Orders and Emergency Action Search Screen. You can also request a copy of your final order from our Public Records Unit via the online request form.
Q. Who do I contact regarding Compliance with a Final Order?

Q. How do I prove that I have complied with the requirements of my Final Order?
A. It is your responsibility to provide documentation showing that you have completed the requirements of your Final Order by the due date.
Examples of documentation:
- Copies of Continuing Education Certificates – should be obtained by licensee
- Respondent Report
- Monitor Report
- Community Service Affidavit
*Documents needing to be notarized must be the original.
Note: It is important to retain copies of every document you send to the Compliance
Management Unit indefinitely as you may be asked to provide proof of compliance in the future on applications, for credentialing or insurance.
Please remember that the Final Order and “discipline on file” are public information and will remain on your license indefinitely.
Q. How do I get Board approval for the requirements imposed by the Final Order?

Q. How do I pay fines and costs?

Note: If you are satisfying a dishonored payment, it must be submitted by a cashier’s check or money order.
Q. How do I update/change my Address or Name?

Q. Do I need to renew my license while I’m under discipline?
A. Yes. Your license will need to be renewed while on probation, suspension or obligations.
Q. What does my license status mean? (i.e. “Obligations/Active”)

Q. How do I appeal my Final Order?
A. Contact your local 1st District Court of Appeals within thirty (30) days of the date your Final Order was filed.
Q. What do I do when Consumer Services notifies me of my failure to comply with my Final Order?
A. Any questions regarding a potential investigation or referral should be directed to the Consumer Services Unit.
Note: Any submission of documentation should be submitted to both the Compliance
Officer and the Consumer Services’ Investigator that notified you.
Q. What does a consumer see when they look up my license?
