Important Update Regarding Background Screening Fees
Effective March 19, 2012, the cost of the national check portion of criminal history requests was REDUCED from $19.25 to $16.50.
The FBI periodically reviews the amount of fees charged for the national portion of criminal history record checks and is using this current review as the basis for revising their user fees.
The state portion of fees for Florida criminal history requests are legislatively mandated and will remain the same.
What does this mean for you the applicant?
The total cost of the background check for licensure has been reduced from $48.00 to $43.00.

Initial Licensure Fee Information Breakdown of Fees
Upon the Board's certification of an application for licensure, the applicant is notified of the licensure fee and requisites for licensure that include affidavits related to:
- Financial Responsibility (Financial Responsibility is malpractice insurance as defined by Florida Statutes)
- HIV/AIDS
- Dispensing Practitioner Status
- NICA Assessment.
1. Biennial Licensure Fee $424.00 (this fee is not prorated)
2. Unlicensed Activity Fee $5.00
3. Florida Birth-Related Neurological Injury Compensation Assessment (NICA):
- Participating Physician $5,000.00*
- Non-Participating Physician $250.00*
*Note: If you qualify for a NICA Exemption, no additional assessment is required.
NICA Exemptions:
- Resident physicians, assistant resident physicians and interns in postgraduate training programs approved by the Board of Medicine
- Retired physicians who maintain an active license, but who have withdrawn from the practice of medicine
- Physicians who hold a limited license, as defined by Chapter 458, F.S., who do not receive any compensation for medical services
- Physicians employed full-time by the Veterans Administration whose practice is confined to VA hospitals
- Any licensed physician on active duty with the Armed Forces of the United States
- Physicians employed full-time by the State of Florida whose practice is confined to state-owned correctional institutions and state-owned mental health facilities
