Occupational Therapy Assistant (OTA) application & licensure fee $180
OT and OTA application fee $100
OT and OTA initial licensure fee $ 75
OT and OTA unlicensed activity fee $ 5
These fees are for Florida licensure only. There will be examination fee
charges imposed by the National Board for Certification in Occupational
Therapy, Inc., (NBCOT).
The $180 fee must accompany the application or the application will not be
processed.
The licensure fee ($75) may be refunded to you if you are denied licensure
or if you decide to withdraw your application.
Please make your personal check or money order payable to the Department of
Health and send the fee, application and appropriate documentation to:
Department of Health, Board of Occupational Therapy Practice, P.O. Box 6330,
Tallahassee, Florida 32314-6330.
Should your check be returned due to insufficient funds, application
processing will stop. Processing will resume once the outstanding funds and
service charges are covered. Your account will be turned over to a
collection agency should you not rectify the matter of any insufficient fund
