General Information:
-
License must be renewed every two years - expires
December
31 of each even year.
-
Failure to receive application for renewal does not
relieve a licensee of the responsibility to renew.
-
20 credit hours of approved continuing education are
required per biennium
-
Do not send in CE information. Licensee shall retain
documentation showing completion of CE requirements for a period of 4
years from the beginning of the biennium for which credit is claimed.
-
Random audits will be conducted to assure CE
requirements are met.
-
Licensee may elect at renewal time to place license in
inactive status. Inactive licenses must be renewed biennially including
payment of renewal fee.
Re-Instatement of Delinquent License:
-
Submit a written request for either active or
inactive status.
-
Submit either the active status or inactive status fee,
which ever is applicable.
-
Submit the delinquent status fee and, if applicable, the
change of status fee.
-
Meets all continuing education requirements of Rule
64B12-15.001, if electing active status.
Re-Activation of Inactive License:
-
Submit the reactivation fee and, if applicable, change
of status fee.
-
Submit a written request for reactivation.
-
Submit the applicable renewal fee,
-
If requesting reactivation during the licensure
renewal cycle, submit the active renewal fee.
-
If requesting reactivation outside the renewal
cycle, submit the difference between the inactive renewal fee and
the active renewal fee, plus the change of status fee.
-
Must meet the continuing education requirements as
set forth in Rule 64B12-15.001, for each biennium the license was in
inactive status.
