Online Prescription Department Manager/Consultant Change Request
Section 465.022, Florida Statutes requires the permittee to notify the department within 10 days after any change in prescription department manager or consultant pharmacist of record, and that no pharmacist may be registered as the prescription department manager of more than one pharmacy unless approved by The Board of Pharmacy.
You may now add or remove your license as PDM or Consultant of record online by:
- Go to Practitioner Login
- Select Pharmacist or Consultant Pharmacy profession
- Enter your user id and password
- Select Manage Related Licenses and add or delete your relationship to the pharmacy
You must have the pharmacy license number in order to update the record. You may verify the relationship online at Look Up a License by looking up the Pharmacy license number. Your name should be added or removed under the supervising practitioner title.
