Apply Online for Licensure: Eliminate mailing time and expedite your application! Apply online, using your user id and password, and receive a faster response!

Note: If you have previously submitted an application to the board office, you cannot complete the on-line application. You must submit a paper application through the mail for any retakes or expired files. Please obtain the correct application by clicking the Applications and Forms button above.

Continuing Education: Click here for information regarding continuing education requirements for licensure.   It is also strongly recommended that you review all of the laws and rules regulating this profession.

Accommodations: If you need to request Special Testing Accommodations for your exam(s), please review the material available from Examination Services.

Exam: All applicants are required to take and pass the Florida Laws and Rules Exam.  Applicants applying by examination must also register to sit for the National Physical Therapy Exam (NPTE) prior to submitting an application. Both exams are given by the Federation of State Board of Physical Therapy (FSBPT).  Visit FSBPT's website for registration and other information.

Endorsement: Applicants applying by endorsement must have passed the NPTE and hold an active license in another state.  In addition, all applicants are required to take and pass the Florida Laws and Rules Exam.  The Florida Laws and Rules Exam is given by the Federation of State Board of Physical Therapy (FSBPT).  Visit FSBPT's website for registration and other information.
