Credentialing - CoreStat - Updates, content, or to obtain a form: Physician Credentialing

Effective July 1, 2002, the health care practitioner credentialing program will no longer require practitioners to report core credentials data to the department due to the repeal of s. 456.047, F.S. The data collected by the department will be accessible online to existing subscribers and practitioners. 

Subscribers may continue accessing the data until their subscription expires. The department will not issue access to new subscribers. Practitioners may continue accessing and updating the practitioner profile data using the system.
