Community Health Status Assessment Exercise Directions

The Community Health Status Assessment (CHSA) answers the question, “How healthy are our residents?” and “What does the health status of our community look like?” The results of the CHSA provide an understanding of the community’s health status and ensure that the community’s priorities include specific health status issues.

Directions

Using the county health status profile report from Florida CHARTS:  www.Floridacharts.com the Florida Department of Health public health data website, write a brief description of each of the categories or domains of health listed in the tables on the “County Health Profile Domain Worksheet”.  Next, prepare a brief summary of the health status of the county describing the current health status and possible contributing factors.

There are also a variety of other county data reports on CHARTS which can be used during this exercise.
