Health Improvement Plan
Now that you have identified the strategic
issues you will be working on for the next few years in your community, it
is time to document these issues, goals and activities in a Community
Health Improvement Plan document. A community health improvement plan is
a compilation of individual action plans.
For this exercise, choose one of the three strategic
issues identified by your group. Using the information you recorded on the

in the previous section, you will now record the information on the
