Order a Florida Vital Record Certification Online
(Birth, Death, Marriage and Divorce Records)
Vital record
certificates can be ordered online using a credit card through VitalChek, the
only Florida Vital Statistics contracted online service agency. Orders may also
be placed through VitalChek by calling toll-free 1-877-550-7330. These orders
are considered RUSH requests. A $10.00 RUSH fee will be charged for each online
order, in addition to VitalChek's $7.00 credit card service fee. UPS charges may
also apply.
**If you do not
need rush service, you may order from our office Monday through Friday from 8:00
a.m. until 4:30 p.m., and phone service until 5:00 p.m., excluding
holidays. If you are requesting a certified copy, the only fee you will pay will
be the search fee. We are located at 1217 N Pearl Street, Jacksonville, Florida

Eligibility requirements for certifications of
birth or
death with cause apply.
Any person who willfully and knowingly provides any false information on a
certificate, record or report required by Chapter 382, Florida Statutes, or on
any application or affidavit, or who obtains confidential information from any
Vital Record under false or fraudulent purposes, commits a felony of the third
degree, punishable as provided in Chapter 775, Florida Statutes.
