The Bureau of Vital Statistics uses the VitalChek® Network as our ONLY contracted vendor to accept mail, phone, fax and internet orders billed to a credit card.  Go to the VitalChek® website or speak with a customer representative to place your order by telephone at 1-877-550-7330. If you should experience problems with the online ordering, please call VitalChek® for assistance.  Please note that some of the forms below are not available through VitalChek®  such as the Putative Father forms.  Those forms must be mailed directly to the state office to be processed.

 

The majority of the Vital Statistics Forms can be easily completed online using your computer and then printed for submission. You can mail or fax the completed form to the Bureau of Vital Statistics. The mailing address and fax number are both located on each form. The applications listed below apply only to Florida records.

Fetal Demise Form

This form can be completed online from your computer. After completing the form please print, sign and mail to the address on the form.  When mailing please send any associated fees with your completed application.  This form is for use by physicians to notify a mother of options available for disposition

of fetal remains.

 

Brochures                       

 

This brochure is designed to assist prospective parents by providing information they need to know

prior to the delivery of their child, such as naming the child, father information when the mother is

unmarried, healthy start, obtaining the child's social security number, etc. Copies are also available

Este folleto está diseñado para ayudar a los futuros padres por proporcionar la información
